Maybe it's the threads on the flywheel nut. It may be an idea to put a new clutch nut and flywheel nut on at the same time.
Another possibility is the clutch spring you are using or the adjustment of the spring nut. Are you running the clutch very loose? ie. less than 0.8 to 1mm thread showing on flywheel nut. The tension on the spring helps to lock the nut on the threads.
You could put some locktight on the nut. On our first car (not Xray) it suggested using locktight. However, this has not been necessary with our NT1 car and will make clutch adjustment more difficult so this would be a last option.
